I cannot comment on the quality of services provided here. This is because I refused to sign the following mentioned document. "Assignent of Benefits Coastal Ear, Nose and Throat" that states "I hereby irrevocably, designate, authorize and appoint Provider listed above as my true and lawful attorney-in-fact." The document goes on to describe the (self-described) "limited purpose" of this. I'm no lawyer, but I should not have to feel the need have one consult on a document such as this so i can have a diagnosis of sleep apnea confirmed. My social security number would be required before I would be seen. My insurance is by a major company and verifiable. In the last year I have had two surgeries: one major, one minor. Neither of those surgical practices required these submissions.
